Visa Forms
From March 1, 2010, all applicants including children, must complete their own DS-160
online visa application form before coming for their interview. The Embassy / Consulate
will not accept older applications that have been handwritten or typed.
Your DS-160 application form is electronically sent to Embassy / Consulate,
but you must print and bring the DS-160 confirmation page to the Embassy/Consulate.
Be sure to select the proper location (Abuja or Lagos) when completing your online application.
Note: When you electronically sign your DS-160, you are certifying that all the
information contained in it is true and correct. Misrepresenting any facts could render you
ineligible to enter the United States. Please double-check that all your answers are accurate
and that every question is answered completely before you submit the form.
